Oil pulling (kavala or gandusha in Sanskrit) is described in the Charaka Samhita and Sushruta Samhita — two foundational Ayurvedic texts written approximately 2,000 years ago. The practice involves swishing oil in the mouth for 15–20 minutes, then spitting it out. Sesame oil is the traditional medium.

For most of its history, oil pulling was dismissed by Western medicine as folk remedy without scientific basis. Over the past two decades, that has changed. Multiple randomised controlled trials have found that oil pulling with sesame oil significantly reduces oral bacteria, plaque, and gingivitis — with an effect size comparable to chlorhexidine mouthwash.

Here's what the research shows and how to do it properly.

The Science Behind Oil Pulling

Mechanism 1: Saponification

When oil is swished vigorously in the mouth, it undergoes saponification — a chemical reaction with saliva that creates a soap-like emulsion. This emulsion has detergent properties that break down the lipid membranes of bacteria, killing them and removing them from tooth surfaces and gum tissue.

Mechanism 2: Adhesion

Bacteria in the mouth have lipid-based cell membranes. Oil is lipophilic (fat-loving) — it attracts and binds to these bacterial membranes. When you spit out the oil after pulling, you're removing the bacteria that have adhered to it.

Mechanism 3: Sesamin's Antimicrobial Properties

Cold pressed sesame oil contains sesamin — a lignan with demonstrated antimicrobial activity against Streptococcus mutans (the primary bacteria responsible for tooth decay) and other oral pathogens. This is specific to sesame oil; other oils used for pulling (coconut oil, sunflower oil) don't have this compound.

Clinical Evidence

Study 1 (2009, Journal of Oral Health and Community Dentistry): 20 adolescents with plaque-induced gingivitis performed oil pulling with sesame oil for 45 days. Results: significant reduction in plaque index, modified gingival index, and total colony count of aerobic microorganisms. Effect comparable to chlorhexidine mouthwash.

Study 2 (2011, Journal of Indian Society of Pedodontics and Preventive Dentistry): Oil pulling with sesame oil reduced Streptococcus mutans counts in saliva by 20% after 2 weeks. Chlorhexidine reduced counts by 33% — oil pulling was less effective but had no side effects (chlorhexidine causes tooth staining and taste disturbance with prolonged use).

Study 3 (2014, Nigerian Medical Journal): Oil pulling with sesame oil reduced plaque and gingivitis scores significantly over 4 weeks, with results comparable to chlorhexidine.

Study 4 (2016, Journal of Traditional and Complementary Medicine): Systematic review of 7 studies concluded that oil pulling with sesame oil is effective for reducing plaque and gingivitis, with a good safety profile.

How to Do Oil Pulling Correctly

What You Need

  • 1 tablespoon cold pressed sesame oil
  • 15–20 minutes in the morning

Step-by-Step

  1. Do it first thing in the morning — before eating, drinking, or brushing. The bacterial load in the mouth is highest after sleep.

  2. Take 1 tablespoon of cold pressed sesame oil into your mouth. Don't swallow.

  3. Swish gently — push and pull the oil through your teeth, around your gums, and across your tongue. The motion should be gentle, not vigorous. You're not gargling.

  4. Continue for 15–20 minutes. This is the minimum time needed for the saponification and adhesion mechanisms to work. Set a timer.

  5. Spit into a bin — not the sink. The oil will solidify and clog drains over time.

  6. Rinse your mouth with warm water.

  7. Brush your teeth as normal.

Common Mistakes

Using too little oil: 1 tablespoon is the minimum. Less oil doesn't provide enough volume to reach all surfaces.

Swishing too vigorously: This causes jaw fatigue and doesn't improve results. Gentle swishing is sufficient.

Stopping too early: 5–10 minutes is not enough. The 15–20 minute duration is important for the mechanisms to work.

Swallowing the oil: The oil absorbs bacteria and toxins during pulling — swallowing it defeats the purpose. If you feel the urge to swallow, spit and start again with fresh oil.

Using refined sesame oil: Refined sesame oil has reduced sesamin content. Cold pressed sesame oil is the traditional and more effective choice.

Which Oil Is Best for Oil Pulling?

Cold pressed sesame oil is the traditional Ayurvedic recommendation and has the most clinical evidence. The sesamin content provides antimicrobial activity specific to oral pathogens.

Cold pressed coconut oil has become popular in recent years due to its lauric acid content, which has antimicrobial properties. Some studies show comparable results to sesame oil. It's a valid alternative, particularly for those who find sesame oil's flavour unpleasant.

Cold pressed sunflower oil was used in some early studies. It works through the mechanical mechanisms (saponification, adhesion) but lacks the specific antimicrobial compounds of sesame or coconut oil.

The verdict: Cold pressed sesame oil is the best choice based on traditional use and clinical evidence. Cold pressed coconut oil is a good alternative.

What Oil Pulling Can and Cannot Do

Can Do:

  • Reduce oral bacteria (Streptococcus mutans, Lactobacillus)
  • Reduce plaque accumulation
  • Reduce gingivitis (gum inflammation)
  • Freshen breath (by reducing bacterial load)
  • Complement regular brushing and flossing

Cannot Do:

  • Replace brushing and flossing
  • Treat established tooth decay
  • Whiten teeth (despite popular claims — no clinical evidence)
  • Detoxify the body (a popular but unsubstantiated claim)
  • Treat systemic diseases

Oil pulling is an oral hygiene practice, not a systemic health treatment. The claims about it "pulling toxins from the blood" are not supported by evidence.

Practical Integration

Oil pulling takes 15–20 minutes — which is a significant time commitment for a morning routine. The most practical approach:

  • Do it while showering, getting dressed, or reading the news
  • Start with 5 minutes and gradually increase to 15–20 minutes over 2 weeks
  • Aim for daily use; even 3–4 times per week provides meaningful benefits

Most people who establish the habit report noticeably cleaner-feeling teeth and fresher breath within 2 weeks.
